A chain-of-custody record will be filled out for each shipment of samples to be sent to the laboratory for analysis. Information contained on the triplicate carbonless form will-include the following: 4 o Date and time the sample was taken: For QA/QC samples, such as replicates and splits, the actual sample time shall be recorded, not the time of the primary sample. O Sample number and the number of sample bottles. o Analyses required. O Remarks including preservatives added and any special conditions or specific quality control measures. o Container number in which sample had been packaged. O Labeling of travel blanks. Blank spaces on the chain-of-custody record will be crossed out between the last sample number listed and the signatures at the bottom of the sheet. After carefully packaging the samples into numbered containers (e.g., ice chests) for transfer to the laboratory, the field sampler will sign the chain-of-custody record and record the time and the date. The original imprint of the chain-of-custody record will accompany the sample L containers. Following review by the project manager or task leader, a duplicate copy will be placed in the project file. 5.0 EQUIPMENT CLEANING Sample bottles and bottle caps will be cleaned by the subcontracted laboratoryusing standard EPA approved protocols. Sample bottles and bottle caps will be protected from contact with solvents, dust, or other contamination between time of receipt by B&R and time of actual usage at the sampling site. Sampling equipment that will be reused shall be cleaned/decontaminated after sampling of each well. Thermometers, pH electrodes, and conductivity probes that will be used repeatedly will be;cleaned after sampling of each well and at any time during sampling if the object comes in Lcontact with foreign matter.
