Page 2, the Licensed Contractor Declaration <br /> (LCD) gets filled out by the C-57 driller. If the C-57 driller does NOT sign page 1, then the C- <br /> 57 driller MUST sign the bottom section of the LCD authorizing whomever signed page 1. <br /> I can waive a 'prepayment' charge since I have an open file for the USGS and these 'all-over- <br /> the-County' wells. new sites must pre-pay for 3-hours of start up fees at $125/hour ($375) <br /> AND turn in another MFR if the billing party on the other MFR is to be different. But you don't <br /> have to send in this $375, unless.... <br /> Currently this site has a billing party on record: John Izbicki, USGS, Interior Dept, 4165 <br /> Spruance Rd, #200, SD.
