Waste oil—the annual amount element has not been completed. <br /> 2. Propane—the storage pressure is wrongly identified as "ambient"vs. "above ambient". <br /> 3. Sealant compound—the hazardous materials type element has not been completed. <br /> 4. Hydrogen gas-the storage pressure is wrongly identified as "ambient"vs. "above ambient". <br /> 5. Carbon dioxide -the maximum daily amount and size of the largest container is under reported at 1,200 <br /> cubic feet (the capacity of the tank is likely 300 pounds; carbon dioxide can be reported in pounds). <br /> • Violation #6,the site map does not show or address the following: <br /> 1. Storm and sewer drains. <br /> 2. The location of the waste oil, propane, sealant compound, hydrogen gas and sodium hypochlorite, and <br /> carbon dioxide. <br /> 3. The Evacuation staging area does not correlate with the location identified in the Section E. Facility <br /> Evacuation of the Emergency Response/Contingency Plan. <br /> 4. A legend to identify the codes/symbols used on the map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> • Emergency Response/Contingency Plan -the Emergency Response/Contingency Plan may not be accurate. <br /> 1. Section E. Facility Evacuation -the location of the Emergency Assembly areas does not correlate with the <br /> location shown on the site map. <br /> • Employee Training Plan—the employee training plan and been replaced by employee training records. <br /> - A training plan must address training for all new employees and annual training, including <br /> refresher courses,for all employees in safety procedures in the event of a release or threatened release <br /> of a hazardous material, including, but not limited to: <br /> 1 <br />
