I was inspected last year by John Alaniz(CUPA) inspector for our facility from county HazMat waste. Inspecting our blast media, he stated our analytical was missing a document, I thought I forgot to give him a "Chain Of Custody" page. Not the case, I did give him that, He stated it was missing a quality control page from the lab. I am attaching the packet I gave him. Do you have a copy of that page? Or the lab possibly?
