(209)931-2227 Fax <br /> August 29,2019 Project No.4-419-0381 <br /> HEALTH AND SAFETY PLAN <br /> PROPOSED PETROLEUM STATION AND QSR <br /> 760 WEST CHARTER WAY <br /> STOCKTON,CALIFORNIA <br /> 1.0 INTRODUCTION <br /> This document describes the health and safety procedures for the field activities planned during <br /> implementation of soil collection and stockpile management activities to be conducted at the Proposed <br /> Petroleum Station and QSR located at 760 West Charter Way in Stockton,California(the Site).The location <br /> of the Site is shown on Figure 1. <br /> All SALEM Engineering Group, Inc. (SALEM)personnel will follow this plan. Subcontractors may wish <br /> to use this plan as a guideline; however, subcontractors are encouraged to follow a health and safety plan <br /> developed by their respective companies. The primary responsibility for employee safety lies with each <br /> company for its own employees. However, each field team member and the separate companies working <br /> on this project will maintain a general responsibility to identify and correct any health and safety hazards <br /> and to cooperate toward working as safely as possible. <br /> 1.1 Project Description <br /> The scope of the site investigation work consists of advancing 5 soil borings to approximately 20 feet below <br /> ground surface(bgs)for the purpose of collecting soil and groundwater samples.Borings will be completed <br /> using a Strataprobe direct-push rig operated by a subcontractor (TEG). Constituents of concern include <br /> petroleum hydrocarbons, volatile organic compounds (VOCs), fuel oxygenates, polynuclear aromatic <br /> hydrocarbons (PAHs), metals and polychlorinated biphenyls (PCBs). It is anticipated that the scope-of- <br /> work will be completed within a 1 day period and will entail the on-site presence of one SALEM employee. <br /> 1.1.1 General Health and Safety Considerations <br /> SALEM personnel and subcontractor employees will comply with all Federal OHSA,and California OSHA <br /> health and safety regulations. The particular hazards associated with the project area are discussed in <br /> subsequent chapters of this plan. <br /> 1.2 Personnel Responsibilities <br /> Project personnel who will have responsibility for the operation of this project are: <br /> Project Administrator: Richard Opp, SALEM <br /> Project Manager and Site Safety Officer: Shannon Lodge, SALEM <br /> SALEM Field Team Members: Shannon Lodge, SALEM <br /> Jim Robert, SALEM <br /> Subcontractor Team Members: TBD <br /> Field personnel will be required to have successfully completed health and safety training prior to any site <br /> work. SALEM personnel have completed the OSHA 40 hour course, OSHA 8 hour supervisors' training, <br /> and annual OSHA 8 hour refresher courses as specified by 29 CFR 1920.120 and Title 8 CCR, Section <br /> LOS ANGELES • SAN JOSE FRESNO STOCKTON BAKERSFIELD <br /> DALLAS 0 SEATTLE 0 DENVER <br />
