To request a <br /> waiver, complete the enclosed "Permit Waiver Request" form and return with any additional <br /> information requested below. <br /> Complete and return page 2 of the application. Were USTs 1 and 2 replaced? You state the <br /> USTs were removed in May 1999. Provide documentation from San Joaquin County <br /> Environmental Health Division that you complied with the December 22, 1998 upgrade <br /> requirements. Provide a description of all activities related to the unauthorized release. Provide <br /> a copy of a site map with a north arrow that shows the location of the former USTs and any <br /> current USTs. <br /> You are required to demonstrate financial responsibility. The "Certification of Financial <br /> Responsibility" and "Chief Financial Officer Letter" forms that you provided are dated June 3, <br /> 1994. This is not acceptable. Enclosed are forms and instructions.
