The boreholes <br /> will be backfilled using hydrated bentonite chips/pellets. <br /> 3 WELL DESTRUCTION ACTIVITIES <br /> All field activities will be performed in conformance with the site-specific health and safety plan <br /> (Appendix B, Health and Safety Plan). <br /> 3.1 Pre-Destruction Activities <br /> A well destruction permit application will be submitted to the County EHD upon approval of this <br /> delineation and destruction work plan. Prior to the commencement of field activities, a private <br /> underground-utilities-locating company will perform a survey to determine whether underground <br /> utilities are present near the sample locations. In addition, Dudek will contact USA North 811 at <br /> least 2 days prior to subsurface work. <br /> Dudek will contact the site owner to arrange for site access, which may require Premium Truck <br /> Repair to move two or more trucks/trailers to accommodate the drill rig. Dudek will give the site <br /> owner as much notice as possible to make the necessary arrangements. <br /> The well will be destroyed by a California-licensed C-57 contractor. <br /> 10886-7 <br /> DUDEK 8 January 2019 <br />
