Time In: 9:20 am <br /> r <br /> Time Out: 10:06 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: CARLS JR UNIT#319/7482 Date: 07/06/2020 <br /> Address: 820 W KETTLEMAN LN, LODI 95240 <br /> Owner/Operator: WOMAR INC Telephone: (209)333-1288 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00052180 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THEY WERE SEVERED MOLDY BREAD. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went the facility and spoke to the Assistant manager who was aware of the <br /> complaint. <br /> Buns come to the facility 3 times a week(Saturday, Monday, and Wednesday)and are checked daily by employees for"use <br /> thru"dates and freshness. I observed buns and buns are within the"use thru"dates. <br /> Buns come to the facility on racks that are a minimum of six inches off of the floor. <br /> Buns on racks are kept in the back storage area near storage racks. <br /> Buns that are ready for immediate use are kept on racks near biscuit prep area. <br /> All buns are kept in the original plastic bags until food is ordered. <br /> I observed "use thru"dates for buns in both storage areas, I also observed a small, large, and a honey wheat buns. <br /> Buns did not appear moldy at this time, and I did not observe cross contamination in bun storage areas. <br /> Once ordered, employee removes bun from original plastic bag, opens bun and places on the char broiler. <br /> Assistant manager showed me this process. <br /> 'Moldy bread"was notobserved during this compliant investigation. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Juana Calderon,Asst Manager, Sig n <br /> EH Specialist: STEPHANIE RAMIREZ Phone: (209)468-9851 <br /> FA0000532 C00052180 SCO04 07/06/2020 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
