<br /> Attached are San Joaquin County Environmental Health Department's Well &Boring Permit
<br /> Application,Licensed Contractors Declaration,Master File Record, and a check for$2,125 for
<br /> abandoning Diamond Pet Food Processors of Ripon, LLC's monitoring wells OB-3, OB-6, OB-
<br /> 10, OB-12, OB-13, OB-14, OB-15, OB-16, OB-18, OB-20, TH-2, TH-4, TH-5, and TH-6,
<br /> located at 942 South Stockton Avenue, Ripon California(APN 259-34-012; Figure 1).
<br /> Table 1 (following text)presents wells construction details,including casing size and screen
<br /> intervals for all the wells.
<br /> Copies of Lawrence &Associates' Groundwater Monitoring Well Destruction Plan, dated April
<br /> 26, 2013; a copy of the California Regional Water Quality Control Board, Central Valley
<br /> Region's (CVRWQCB) letter dated September 30, 2013, approving the scope of work proposed
<br /> in the well destruction plan have previously been submitted to you with the applications to
<br /> abandon monitoring wells OB-27 and TH-3 so they have not been attached to this application.
