I have attached the completed Well & Boring Permit Application/Licensed Contractors Declaration (LCD)and the Master File Record Information (MFR)for abandoning monitoring well TH-3 located on APN 259-34-009. Please give them a look and let me know if they are filled out correctly. If they look okay, I will complete the forms for the other wells to be abandoned on the adjacent parcel and mail them all to your attention. I also will include (1)a copy of our work plan, (2)the Water Board's letter approving the work plan, (3) copies of the well completion logs(they need to be attached, right?), and (4)two checks,one for each parcel. I have one well located in the right-of-way of a street and I will submit a third set of documents for its abandonment once I have received an encroachment permit from the city of Ripon.
