Samples will be retained in appropriate sample bottles provided by the testing <br /> laboratory with the proper preservatives. <br /> Samples will be stored and transported in iced coolers at a temperature not to exceed 4° C for <br /> delivery to the laboratory. <br /> The samples will be delivered accompanied by chain-of-custody forms similar to the format <br /> shown in the Appendix. <br /> 7. REPORTING <br /> WDR Order No. R5-2012-0105 and the associated monitoring and reporting plan require that <br /> Detection Monitoring Reports are to be submitted on a quarterly and annual basis.
