Box 806 Sacramento,CA 95812-0806 Re: Missing Designated Facility Signature for Manifest 003732220GBF (4-17-2018) Dear Department of Toxic Substances Control, We recently had a routine inspection for our operating permit with San Joaquin County Environmental Health Department. During this routine inspection, it was brought to our attention that Manifest 003732220GBF did not have a signature and date from the Designated Facility. We first contacted Transporter 1, American Valley Waste Oil, Inc., and spoke with Hannah Britton. Hannah stated that their Company did not have record with a signature in the Designated Facility section. We then proceeded to contacted Transporter 2, Ashbury Environmental Services, multiple times but were unable to directly speak with someone. We finally contacted the Designated Facility,Yuma Yes LLC,and spoke with Sherry Martinez.Sherry stated that their Company had both signatures and dates from Transporter 1 and 2 under line 17 Transporter Acknowledgment of Receipt of Materials but did not have a signature and date under Designated Facility section. At this point,we feel that we have done our due diligence on contacting the appropriate companies to obtain a copy of the manifest with all required signatures. We have been advised by the inspector with the San Joaquin County Environmental Health Department to inform DTSC Report Repository, Generator Information Services Section,that a signed copy by the designated facility was not received by the generator.
