N%000 Monte Vista Entitlement,Inc. <br /> �. <br /> Phase I ESA Peer Review and Limited Phase lI ESA Report <br /> November 18,2003 <br /> Page 2 <br /> �• Phase I ESA Peer Review and Review of Sanborn Fire Insurance Ma s <br /> Condor reviewed the Phase I ESA report dated December 2000 prepared by Geocon Consultants, Inc. for <br /> the site. The report indicated that the Police Department was conducting police dog training in the <br /> basement of the building at the time of the site reconnaissance. This may have interfered with the <br /> reconnaissance of the basement. The report indicated Sanborn Fire Insurance Map information for parcel <br /> 149-180-07 was not available for the 1917 and 190 maps. <br /> Condor reviewed Sanborn Fire Insurance Maps for the years 1895, 1917, and 1950 through the San Jose <br /> Public Library Internet site. Coverage was available for all six parcels on all three maps. No USTs or <br /> I" aboveground storage tanks were identified on any of the site properties or any adjoining properties on any <br /> of the Sanborn Fire Insurance Maps. <br /> V Site Reconnaissance <br /> Condor conducted a site reconnaissance on the sidewalks in front of each parcel. Potential UST fill <br /> connections were noted on site in the sidewalk on the west side of parcel 149-200-01 and on the north <br /> L" side of parcel 149-200-02. An apartment building is located on the parcel immediately north of parcel <br /> 149-200-10. The south side of the apartment building is on the property line. A potential UST fill <br /> connection was noted off site in the sidewalk on the w st east of the apartment building and immediately <br /> �. north of the property boundary with parcel 149-200-i0. A 3/,inch diameter pipe that may be a tank vent <br /> was attached to the wall of the apartment. A slumped area with an asphalt patch near the center of parcel <br /> 149-200-09 was noted as a potential UST or former UST location. <br /> La <br /> A reconnaissance of the basement of the buildings on parcels 149-200-01 and 1.49-200-02 was conducted <br /> on July 31, 2002. The St. Vincent's Thrift Shop is located in the buildings. Mr. Bill Rapinsky of the Thrift <br /> LW Shop provided access to the basement and accompanied Condor personnel during the reconnaissance. Six <br /> alcoves with approximately four foot by four foot openings in the concrete floor were present in the wall <br /> on the north side of the basement. The opening in the alcove in the northwest corner of the basement <br /> contained soil with an apparent oily residue. <br /> L. <br /> An abandoned elevator shaft located near the south wall of the basement contained debris that may <br /> contain asbestos. An active elevator near the southeast corner of the basement is operated by a hydraulic <br /> +� lift. Hydraulic oil was present at the base of the hydraulic lift. <br /> No evidence of fuel lines or fuel storage tanks was noted in the basement. <br /> Lo <br /> Mr. Rapinsky pointed out a pipe that he believes may be a UST vent on the outside of the east wall of the <br /> building on parcel 149-200-03. He indicated the vent may be connected to a UST in the parking lot of <br /> parcel 149-200-10 south of the location of the previously removed UST. <br /> Limited Phase 11 ESA <br /> 4 A ground penetrating radar (GPR) study was conducted on the sidewalks in front of each parcel, on the <br /> slump area in parcel 149-200-09, and near the northeast corner of parcel 149-200-10 in the area of the <br /> potential UST on the adjoining property. The results of the GPR survey are presented in a letter report, <br /> Ground-Penetrating Radar Investigation, dated August 1, 2002. A potential UST was identified beneath <br /> the sidewalk in front of the apartment building immediately north of parcel 149-200-10. A potential UST <br /> was identified beneath the sidewalk in front of parcel 149-200-01. This potential UST is located next to <br /> �. the potential UST fill connection identified during the site reconnaissance. The slumped area with the <br /> asphalt patch on parcel 149-200-09 was surveyed. The GPR data indicate this area correlates to a former <br /> excavation that was backfilled. <br /> w <br /> W CONDOR <br /> LO <br />
