The following is an itemized list of underground storage tank violations that have not been addressed for GSG GAS & MART as of May06 , 2020 . Open violations from June 24, 2019 inspection Violation #206 - Failure to comply with UST operating permit conditions . The designated operator' s inspection reports and the manager onsite indicated that UDC 3/4 was impacted by a truck in September of 2018 and they are waiting to hear from the contractor to replace the dispenser . A repair permit application was not submitted to the EHD for this service. All repairs must have a permit submitted to and approved by the EHD prior to beginning work . Immediately, have a properly licensed , trained and certified contractor repair or replace the damaged component under permit and inspection of EHD . ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #301 - UST system is not operated to prevent unauthorized release, including spills and overfills . This testing was required to be conducted by October 13 , 2018 . Immediately schedule this testing and provide 48 hours notification to the EHD .
