OBSERVATION: The designated operator employee training for Jatinder Singh was not performed or not current. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a minor violation. 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. OBSERVATION: A current copy of the UST operating permit was not found on site. REGULATION GUIDANCE: A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. CORRECTIVE ACTION: Immediately obtain a copy of the [CURRENT YEAR] permit and retain it on site. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product -tight. OBSERVATION: Seepage was detected below the shear valve in under dispenser containment (UDC) 1/2, indicating a leak in the primary containment. REGULATION GUIDANCE: All primary containment for the UST system must be product tight. CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component(s), under permit and inspection of the EHD if necessary.
