EHD issues permits to drillers/C-57s, not consultants. When <br /> a consultant signs the permit application, the driller must authorize that person to sign on their <br /> behalf. Either have ECA fill in your name and sign our page 2 and get it to me OR, have the <br /> driller/ECA send me a letter with his signature on it saying it's OK for you to sign for ECA. <br /> ONE OR THE OTHER must be sent in. <br /> Second, each permit must have a Master File Record (MFR) . AND new sites like this ALSO <br /> need a MFR for the billing NOT associated with the drill. One of the $366 fees you paid was <br /> for a pre-payment on a new site and the required MFR identifies WHO will pay NON-permit <br /> fees. Since you sent the required permit MFR that listed Lush Geosciences Inc as the billing <br /> party for the drilling fees, if you also plan on taking the site billing responsibilities I can <br /> duplicate this MFR and you won't need to send another MFR. <br /> so, please one of the following: <br /> 1- advise me in an email that Lush will take SITE billing party also OR <br /> 2- have someone else complete, sign, and submit the attached blank MFR to act as the <br /> site MFR. <br /> I cannot complete the processing and forward your packet to the boss w/o all the info here. <br /> the quicker you address this, the quicker I can finish the processing and get it to the boss who <br /> can assign it to an inspector. Whether or not that inspector can make YOUR schedule is <br /> unknown now. <br /> you should know, the EHD advises that you should allow 10 working days for drilling AFTER <br /> we receive a complete permit packet. <br /> 6/2/2011 <br />
