EHD issues permits to drillers/C- <br /> 57s, not consultants. When a consultant signs the permit application, the driller <br /> must authorize that person to sign on their behalf. Either have ECA fill in your <br /> name and sign our page 2 and get it to me OR, have the driller/ECA send me a <br /> letter with his signature on it saying it's OK for you to sign for ECA. ONE OR <br /> THE OTHER must be sent in. <br /> Second, each permit must have a Master File Record (MFR) . AND new sites like <br /> this ALSO need a MFR for the billing NOT associated with the drill. One of the <br /> $366 fees you paid was for a pre-payment on a new site and the required MFR <br /> identifies WHO will pay NON-permit fees. Since you sent the required <br /> permit MFR that listed Lush Geosciences Inc as the billing party for the drilling <br /> fees, if you also plan on taking the site billing responsibilities I can duplicate this <br /> MFR and you won't need to send another MFR. <br /> so, please one of the following: <br /> 1- advise me in an email that Lush will take SITE billing party also OR <br /> 2- have someone else complete, sign, and submit the attached blank MFR to act as <br /> the site MFR. <br /> I cannot complete the processing and forward your packet to the boss w/o all the <br /> info here. <br /> the quicker you address this, the quicker I can finish the processing and get it to the <br /> boss who can assign it to an inspector. Whether or not that inspector can make <br /> YOUR schedule is unknown now. <br /> you should know, the EHD advises that you should allow 10 working days for <br /> 6/6/2011 <br />
