During a <br /> conversation in the PHS/EHD office on May 3,2000,Mr.Greer acknowledged the meeting date of May 15, <br /> 2000. However,neither of you appeared for the meeting. <br /> In a telephone conversation with Mr.Greer on May 15,2000,he stated that the required encroachment permit <br /> from San Joaquin County Public Works to install monitoring wells on Steinegul Road had been obtained,and <br /> that he intended to comply with PHS/EHD directives. PHS/EHD agreed to extend the deadline for <br /> implementation of the proposed work to June 20,2000. Failure to meet this deadline may result in PHS/EHD <br /> requesting enforcement action by the San Joaquin County District Attorney's office. <br /> According to Appendix B Tri-Regional Board Staff Recommendations,a survey to locate all wells within 2000 <br /> radial feet of the site must be conducted. The addresses of all wells identified in the survey and their locations <br /> plotted on a map,along with any known construction details of the wells should be provided to PHS/EHD. <br /> Submit this well survey no later than September 25,2000.
