Santa Fe Road. <br /> Riverbank,CA.,95367. <br /> In correspondence dated August 31, 1998, San Joaquin County Public Health Services, <br /> Environmental Health Division (PHS-EHD) directed you to install the monitoring wells that were <br /> approved in the work plan submitted in July 1998 and submit the report of findings before <br /> December 1, 1998. The report has not been received. You are hereby directed to submit the <br /> report of findings by June 1,1999. <br /> In addition, monitoring wells are to be sampled on a quarterly basis and the report (Quarter <br /> Monitoring Report[QMR) is to submitted before the next quarterly sampling event. As of this <br /> date, no QMRs have been received. You are directed to quarterly sample all of the monitoring <br /> wells and report the findings to PHS-EHD each quarter. <br /> The QMR for the first quarter(January, February, and March) is past due and is to be <br /> submitted by June 1, 1999. The QMR for the second quarter (April,May,June) is due no <br /> later than July 15, 1999. <br /> In order to become and/or remain eligible to receive reimbursement from the State Water <br /> Resources Control Board (SWRCB)-Clean Up Fund (CUT), sites are required to be in compliance <br /> with directives issued by PHS-EID.
