Santa Fe Road <br /> Riverbank, CA., 95367 <br /> San Joaquin County Public Health Services, Environmental Health Division <br /> (PHS-EHD) has reviewed your August 24, 1998 "Request for Extension of <br /> Drilling and Installation of Monitoring Wells" received on August 27, 1998 and <br /> offers the following comments. <br /> In order to approve a request for an extension, valid reasons for the extension <br /> and an alternate date are to be submitted before the due date provided in the <br /> directive letter. <br /> Your reasons for the extension request were based on the unavailability of the <br /> drilling contractor and that reimbursement has not been received by the State <br /> Water Resource Control Board (SWRCB), Clean Up Fund (CUF). <br /> PHS-EHD's July 22, 1998 directives allowed you sufficient time to contract with a <br /> drilling contractor and secure the drilling date. By submitting the boring permit <br /> application later than the August 7, 1998 deadline, your environmental consulting <br /> contractor did not have adequate time to contract with another C-57 licensed <br /> drilling contractor so that you would be able to meet the deadline established by <br /> PHS-EHD. <br /> In discussions with the SWRCB-CUF staff, PHS-EHD has learned that you will <br /> be issued a Letter of Commitment in the very near future. Your CUF deductible <br /> amount has been set at $10,000.00. To date you have not incurred $10,000 of <br /> expenses so you are not eligible for reimbursement of costs yet. <br /> Additionally, no alternate date was offered in your extension request as required <br /> by PHS-EHD. For these reasons, you and Arrowhead Consulting were notified <br /> in a phone conversation on August 28, 1998 that PHS-EHD disapproved your <br /> request for the extension. <br /> A Division of San Joaquin County Health Care Services <br />
