NAME: ROY F.WESTON <br /> PERMIT t: 1096-6SV-0147 <br /> DATE: May 23, 1996 <br /> permittee shall notify the state representative BILL LaMASCUS, P.O. BOX 2048, Stockton CA, 95201, (209) <br /> 948-3656. All work under this permit must meet with the approval of the state representative. <br /> Failure on the permittee's part to comply with any provision will be cause for revocation of this permit. <br /> IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, PERMITTEE SHALL FILL <br /> OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY GRANTOR. <br /> Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State Standard <br /> Specifications. Reference to Engineer in the State Standard Specifications shall include State Representative. <br /> Permittee is directed to Standard Specification Section 7-1.11 Preservation of Property, and Business and <br /> Professions Code, Section 8771. Permittee shall physically inspect the work site and locate survey <br /> monuments prior to work commencement. Monuments shall be referenced or reset in accordance with <br /> Business and Professions Code. <br /> This permit is not valid until permittee has obtained permission from adjacent property owners, Irrigation <br /> Districts, Cities, Counties or other interested parties to perform the proposed work. <br /> Prior to expiration, permittee shall apply for and obtain a maintenance permit to maintain, monitor and <br /> abandon the well installed under this permit In accordance with State Water Quality Control Board <br /> standards. The maintenance permit may be obtained for a period not to exceed five years from <br /> expiration date of the Installation permit. A deposit in the amount of $280 shall be submitted with the <br /> application for said permit. <br /> Upon completion of work Permittee shall provide Caltrans with a copy of the results from sampling <br /> this well and any Technical Report that is produced. <br /> In addition to the attached General Provisions, the following special provisions are also applicable: <br /> 1. Unless specifically authorized elsewhere in this permit, survey work shall not be performed: <br /> a. On holidays or an afternoon preceding a holiday. <br /> b. In medians. <br /> c. When work requires traffic control on lanes. <br /> 2. Permittee shall furnish, place and maintain required signs, safety equipment, and traffic control and <br /> warning devices in accordance with the Mar.: al of Traffic Controls published by the Department of <br /> Transportation. <br /> 3. All personnel shall wear hard hats and orange vests, shirts or jackets as appropriate. <br /> 4. Unless specifically authorized in this permit, markings within the right-of-way shall be temporary. <br /> Paint used for markings shall be water soluble and other markers shall be removed upon completion of <br /> work. <br /> 5. Any collected data requested by Caltrans shall be furnished to Caltrans without charge. <br /> 6. Vehicles necessary for work may be parked in State Highway right-of-way off paved shoulders. <br /> Auodliary support and employee vehicles shall remain outside the right-of-way. <br /> 7. Permittee shall provide for the safe passage of pedestrians. <br /> Traffic control shall be done in conformance with Caltrans Manual of Traffic Controls 1990. <br /> Page 2 of 2 <br />
