All work under this permit must meet with the approval of the state representative. Failure on the permittee's part to comply with any provision will be cause for revocation of this permit. IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF COMPLETION PROVIDED BY GRANTOR. Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State Standard Specifications. Reference to Engineer in the State Standard Specifications shall include State Representative. Permittee is directed to Standard Specification Section 7-1.11 Preservation of Property, and Business and Professions Code, Section 8771. Permittee shall physically inspect the work site and locate survey monuments prior to work commencement. Monuments shall be referenced or reset in accordance with Business and Professions Code. This permit is not valid until permittee has obtained permission from adjacent property owners, Irrigation Districts, Cities, Counties or other interested parties to perform the proposed work. Prior to expiration, permittee shall apply for and obtain a maintenance permit to maintain, monitor and abandon the well installed under this permit In accordance with State Water Quality Control Board standards. The maintenance permit may be obtained for a period not to exceed five years from expiration date of the Installation permit. A deposit in the amount of $280 shall be submitted with the application for said permit. Upon completion of work Permittee shall provide Caltrans with a copy of the results from sampling this well and any Technical Report that is produced. In addition to the attached General Provisions, the following special provisions are also applicable: 1. Unless specifically authorized elsewhere in this permit, survey work shall not be performed: a. On holidays or an afternoon preceding a holiday. b. In medians. c. When work requires traffic control on lanes. 2. Permittee shall furnish, place and maintain required signs, safety equipment, and traffic control and warning devices in accordance with the Mar.: al of Traffic Controls published by the Department of Transportation. 3. All personnel shall wear hard hats and orange vests, shirts or jackets as appropriate. 4. Unless specifically authorized in this permit, markings within the right-of-way shall be temporary. Paint used for markings shall be water soluble and other markers shall be removed upon completion of work. 5. Any collected data requested by Caltrans shall be furnished to Caltrans without charge. 6. Vehicles necessary for work may be parked in State Highway right-of-way off paved shoulders. Auodliary support and employee vehicles shall remain outside the right-of-way. 7. Permittee shall provide for the safe passage of pedestrians. Traffic control shall be done in conformance with Caltrans Manual of Traffic Controls 1990. Page 2 of 2
