(AGE) for the above referenced site and has the following comments.
The addendum, and the previously submitted "Site Assessment Work Plan — May 1999",
describe the next phase of work recommended for this site. The goal of the work is to define
the lateral extent of the soil contamination to the west and the north of the former tank pit, and to
initiate a groundwater investigation in combination with preparations for feasibility testing.
Three dual-completed monitoring/soil vapor extraction wells will be installed on site. The
monitoring well-screen casing will be installed such that there will be five feet of screen above
the current water table, and fifteen feet below the current water table. Depth to water is
expected to be approximately forty feet below surface grade (bsg). Total depth of the wells is
expected to be fifty-five feet bsg. The soil vapor extraction (SVE) well casings will be screened
from approximately 13-28 feet bsg. Neat cement will be used to fill the annular space between
the bentonite transition seal above the monitoring well filter pack, and the SVE well casings.
At least one additional exploratory boring will be advanced north of the former tank pit area.
This boring should be advanced to a minimum of thirty-five feet bsg to investigate the lateral
extent of the contaminant plume in soil. Field screening will indicate the need to extend the
boring beyond this depth, and the need for another boring further north.
The work plan addendum, as outlined above, is approved. A well permit application has been
previously submitted. Please proceed with submittal of the required $89.00 permit fee, and
notify PHS/EHD within thirty days of the date of this letter that fieldwork has been scheduled.
