W riri l <br /> Southwest, 1[nc. <br /> 6.0 SITE HEALTH AND SAFETY PLAN <br /> A site specific Health and Safety Plan detailing known or potential hazards and emergency <br /> response procedures was prepared prior to field operations. All on-site personnel reviewed the <br /> plan and a "tailgate" safety meeting was conducted prior to initiation of field activities. The plan <br /> was maintained on-site throughout the duration of field activities. <br /> 7.0 DRILL SITE CLEARANCE AND PERMITTING <br /> Prior to the initiation of field operations, the proposed drilling locations were marked in white <br /> and Underground Service Alert (USA) was contacted to identify any potential subsurface <br /> obstructions and/or conflicts. Prior to the well installation work, Mr. Tim Loveland of the Port <br /> of Stockton was contacted and informed of the drilling operation. A check of Port of Stockton <br /> underground maps was made. In order to insure that no underground utilities or obstructions <br /> existed at the proposed drilling locations, the initial five feet of drilling in each location was <br /> conducted utilizing hand auger techniques. No underground obstacles or utilities were identified <br /> by USA, the Port of Stockton, or by the hand augering. <br /> 8.0 SITE ASSESSMENT ACTIVITIES <br /> Three ground water monitoring wells (MW-1, MW-2, and MW-3) were installed on October 8, <br /> 2003 at the locations indicated on Figure 3. The well locations are situated immediately adjacent <br /> to the tank farm to the south, west and north of the spill and product accumulation location.
