SAN J OAQU I N Environmental Health Department <br /> CC LJ N T Y------- <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ACME SAW& INDUSTRIAL SUPPLY 1 1204 E MAIN ST, STOCKTON January 09, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: The following designated facility signed copies of Uniform Hazardous Waste Manifest(UHWM) <br /> numbers were not found on-site: <br /> -003371078GBF(2/2/2017) <br /> -00343452OGBF(8/18/2017) <br /> -003639937GBF(4/5/2018) <br /> -006046285GBF(11/2/2018) <br /> -003733041 GBF(8/3/2018) <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br /> with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. The hazardous waste generator must submit a copy of the manifest <br /> to the California Department of Toxic Substances Control (DTSC)within 60 days if the destination facility signed <br /> manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests and send a copy to the San <br /> Joaquin County Environmental Health Department(EHD). If the destination facility signed manifests cannot be <br /> found then send to DTSC a copy of each generator signed manifest and a statement that the designated facility <br /> signed copy was not received by the generator. DTSC address is DTSC Report Repository, Generator Information <br /> Services Section, PO Box 806, Sacramento, CA 95812-0806. Provide a corrective action statement to the EHD <br /> within 30 days. <br /> This is a repeat violation, Class II. <br /> 113 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION: Copies of consolidated manifest receipts from 1/26/2017, 2/24/2017, 3/23/2017,4/21/2017, <br /> 5/19/2017, 6/14/2017, 7/11/2017, 8/9/2017, 9/12/2017, 10/10/2017, 11/9/2017, 12/6/2017, 1/5/2018, 2/1/2018, <br /> 3/2/2018,4/27/2018, 5/23/2018, 7/18/2018, 8/21/2018, 9/18/2018, 10/16/2018, 11/16/2018, 3/19/2019, 5/8/2019, <br /> 6/4/2019 for spent parts washer fluid were not found on site during the inspection. <br /> Copies of consolidated manifest receipts from 1/31/2017, 12/7/2017, 8/16/2018 for used oil were not found on site <br /> during the inspection <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action regarding <br /> the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Immediately locate a copy of all consolidated manifest receipts and maintain them on site. <br /> Provide a copy of the consolidated manifest receipts and a corrective action statement to the San Joaquin County <br /> Environmental Health Department. <br /> This is a repeat violation, Class II. <br /> FA0004660 PR0514196 SCO01 01/09/2020 <br /> EHD 22-01 Rev.9/20/2019 Page 5 of 8 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
