GROUND ZERO ANALYSIS, INC. <br /> 1714 Main Street <br /> Escalon, California 95320-1927 <br /> Telephone: (209)838-9888 RD IJ tl E <br /> Facsimile: (209)838-9883 IJ tl U <br /> FEB 2 6 2004 <br /> February 24, 2004 ENVIRONMENT HEALTH <br /> PERMIT/SERVICES <br /> Ms. Margaret Lagorio <br /> San Joaquin County Environmental Health Department <br /> 304 East Weber Avenue, Third Floor <br /> Stockton, CA 95202 <br /> Subject: Abandonment of Groundwater Monitoring Wells <br /> Former Southwest Hide Company Facility <br /> 11651 Palm Lane, Manteca, California <br /> Dear Ms. Lagorio: <br /> Enclosed herewith is a completed well destruction permit application along with the associated <br /> fee for the destruction of eleven groundwater monitoring wells at the facility referenced above <br /> (Figure 1). <br /> This work is directed by the California Regional Water Quality Control Board, Central Valley <br /> Region (Regional Board) as part of Waste Discharge Requirements (WDR) Order No. R5-2004- <br /> 0018 for Post-Closure of Southwest Hide Wastewater Ponds, which was adopted by the Regional <br /> Board at a Public Meeting on January 30, 2004. <br /> EHD will be notified at minimum 48 hours prior to conducting fieldwork. <br /> SCOPE OF WORK <br /> A C-57 licensed drilling contractor will destroy wells MW3, MW6, MW7A, MW713, MW9, <br /> MW10, MW11, MW12, MW14, MW18, and MW20 by pressure grouting with neat cement. <br /> Prior to abandonment, the total depth of each well will be measured to ensure no obstructions are <br /> present. Neat cement grout will be pumped into each well through tremie equipment in one <br /> continuous operation until each well is completely full. Additional cement will then be added <br /> under pressure to force the grout through the well screen and into the filterpack, and subsequently <br /> checked for subsidence. If the grout level is observed to have dropped, additional cement will be <br /> added under pressure until it stabilizes. Upon stabilization, the well box and upper three feet of <br /> well casing will be removed by overdrilling.
