GROUND ZERO ANALYSIS, INC. <br /> 1714 Main Street <br /> Escalon, California 95320-1927 <br /> Telephone: (209)838-9888 <br /> Facsimile: (209) 838-9883 <br /> March 27,2003 MAR 2 8 2003 <br /> ENVIRONMENT HEALTH <br /> Ms. Margaret Lagorio PERMIT/SERVICES <br /> San Joaquin County Environmental Health Department <br /> 304 East Weber Avenue, Third Floor <br /> Stockton, CA 95202 <br /> Subject: Abandonment and Replacement of Well MW19 <br /> Former Southwest Hide Company Facility <br /> 11651 Palm Lane, Manteca, California <br /> Dear Ms. Lagorio: <br /> Enclosed herewith is a completed boring permit application along with the associated fee for the destruction <br /> and replacement of well MW 19 at the facility referenced above(Figure 1). <br /> This work is directed by the California Regional Water Quality Control Board, Central Valley Region <br /> (RWQCB) as part of Waste Discharge Requirements (WDR) Order No. 92-077. The well was damaged <br /> during recent surface impoundment closure activities directed by RWQCB and will be destroyed following <br /> local and state guidelines. The replacement well will be installed following local and state guidelines and in <br /> accordance with current industry standards. <br /> EHD will be notified at minimum 48 hours prior to conducting fieldwork. <br /> SCOPE OF WORK <br /> Well Abandonment <br /> A C-57 licensed drilling contractor will destroy the well by pressure grouting with neat cement. Prior to <br /> abandonment, total depth of the well will be measured to ensure no obstructions are present. Neat cement <br /> grout will be pumped through tremmie equipment in one continuous operation until the well is completely <br /> full. Additional cement will then be added under pressure to force the grout through the well screen and <br /> into the filterpack, and subsequently checked for subsidence. If the grout level is observed to have dropped, <br /> additional cement will be added under pressure until it stabilizes. Upon stabilization, the well box and <br /> upper three feet of well casing will be removed by overdrilling. Neat cement grout will be used to backfill <br /> the overdrilled portion to grade.
