' This Health and Safe hPlan (HASP) prescribes the procedures that must be followed during <br /> all project field work. Operational changes which could affect the health or safety of <br /> Personnel, the community, or the environment will not be made without the prior approval of <br /> the IT Project Manager, and the Project Health and Safety (HS) Manager <br /> The provisions of this HASP are mandatory for all IT personnel and subcontractors assigned <br /> ' to the Pacific Bell project. IT requires all visitors to the work site to abide by the <br /> requirements of this HASP The Project HS Manager will provide written addenda to this <br /> ' HASP when changes warrant No changes to the plan will be implemented without prior <br /> approval of the Project HS Manager or his authorized representative. <br /> 1.4 References <br /> This HASP complies with Federal Occupational Safety and Health Administration (OSHA), <br /> California Department of Industrial Relations, Division of Occupational Safety and Health <br /> (Cal/OSHA), United States Environmental Protection Agency (EPA), California EPA <br /> ' (Cal/EPA), and California Department of Toxic Substances Control (DTSC) regulations This <br /> HASP follows the guidelines established in the following documents: <br /> • Standard Qperating Saf Guidelmes (EPA, November 1984); <br /> • Occupational Safety and Health Guidance Manual for Hazardous Waste Site <br /> Activities National Institute of Occupational Health and Safety (NIOSH 86- <br /> 116); <br /> • Title 29 of the Code of Federal Regulations (CFR), Part 1910.120 (OSHA <br /> ' Regulation for Hazardous Waste Operations); <br /> • Title 8 of the California Code of Regulations (CCR), Chapter 4, Subchapter 7 <br /> (commencing with Section 3200) (Cal/OSHA General Industry Safety Orders); <br /> • Title 22 of the CCR, Division 4, Chapter 30 (commencing with Section 66000) <br /> (CaIJEPA, DTSC, Toxic Substance Control Program); and <br /> • Title 22 of the CCR, Division 2, Chapter 3 (commencing with Section 12000) <br />' (California Health and Welfare Agency). <br /> The contents of this HASP are consistent with, or supplement the following IT Health and <br /> Safety Policies and Procedures. <br /> • HS001 Safety Policy <br /> • HS002 Safety Policy: International Operations <br /> • HS010 Employee Safety and Health Work Rules <br /> r HS011 Contractor Safety and Health Rules <br /> m05-W9WSrJ94-0063 tk 1-2 <br /> 1 <br />
