6.0 Site Control <br /> 10 - <br /> Only personnel who have completed 40 hours of hazardous waste operations as defined under <br /> OSHA Regulation 29 CFR 1910.120, have completed their 40-hour training or refresher <br /> training within the past 12 months, and have been certified as fit for hazardous waste <br /> operations by a physician within the past 12 months shall be allowed within a site area <br /> designated as an EZ or CRZ. Personnel without such training may enter the designated <br /> support zone. Only properly trained personnel will be allowed within the EZ or CRZ. <br /> 6.1 Hazard Briefing <br /> MNo person other than the client's operations personnel will be allowed on the site during site <br /> operations without first being given a site hazard briefing. In general, the briefing will <br /> consist of a review of the tailgate safety meeting. All persons on the site, including visitors, <br /> must sign the site-specific tailgate safety meeting form. Tailgate safety meetings shall be <br /> held daily, involving all personnel on site. <br /> 6.2 Documentation of Certification <br /> A subcontractor training and medical file will be established for the project and kept on site <br /> during all site operations. The 40-hour training, update, and specialty training (first- <br /> aid/cardiopulmonary resuscitation [CPR]) certificates, as well as the current annual medical <br /> clearance for all subcontractor personnel, will be maintained within that file. All <br /> subcontractor personnel must provide their training and medical documentation to the FC <br /> prior to the start of field work. Similar documentation for IT employees will be maintained <br /> at the employee's home office. <br /> 6.3 Entry Log <br />■' The IT representative at the site shall record on their Field ActivityDail Lo FADL all <br /> Y g ( } <br /> visitors to the site. These FADLs shall be retained by a site representative for incorporation <br />!; into the project file. <br /> 6.4 Entry Requirements <br /> In addition to the entry requirements listed above, no personnel will be allowed in any EZ or <br /> CRZ unless they are wearing the minimum PPE as described in Chapter 5.0. <br /> MZ105-09-WUST/94-0064 6-1 <br />
