The following is an itemized list of underground storage tank violations that have not <br /> been addressed for Pacific Service Station as of April28, 2020. <br /> Open violations from August 05, 2019 inspection <br /> Violation #121 -Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on August 28, 2018 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by September 27, 2018 . <br /> This information has not been received resulting in a non-compliant status for this facility. An operator that receives <br /> an inspection report shall have 30 days to submit a written response that includes a statement documenting <br /> corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response <br /> documenting corrective actions taken,or proposed is submitted within 30 days of receiving an inspection report. <br /> ❑ This violation was corrected ❑ This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #202 -Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, <br /> 2018). <br /> The Facility Employee Training Certificate did not included the designated operator date of training(section IV). All <br /> site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within <br /> 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate <br /> information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate <br /> information and submit a copy of the updated form to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #209 -Failure to conduct the designated UST operator visual inspection at least once every 30 <br /> days. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -The 05/10/19, 06/07/19, 0705/19 inspection report did not include the signed by the owner/operator. <br /> -The 8/2/19 inspection report was not signed by the owner/operator. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and shall check all required items on the report.
