The following is an itemized list of underground storage tank violations that have not been addressed for Pacific Service Station as of April28, 2020. Open violations from August 05, 2019 inspection Violation #121 -Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on August 28, 2018 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by September 27, 2018 . This information has not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken,or proposed is submitted within 30 days of receiving an inspection report. ❑ This violation was corrected ❑ This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #202 -Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). The Facility Employee Training Certificate did not included the designated operator date of training(section IV). All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #209 -Failure to conduct the designated UST operator visual inspection at least once every 30 days. The designated operator failed to check and note all required items on the visual inspection reports. -The 05/10/19, 06/07/19, 0705/19 inspection report did not include the signed by the owner/operator. -The 8/2/19 inspection report was not signed by the owner/operator. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report.
