My phone notes from September 8 at <br /> 11:16a indicated I advised "Charles" of the fees and forms, our time needed to <br /> process the submittals for the 6 soil borings and soil vapor probes so that he <br /> could obtain a boring permit at a new site. "Charles" did not mention he spoke with <br /> Harlan Knoll as you mentioned. Do you know if Charles spoke with Mr. Knoll <br /> before or after he spoke with me? <br /> As you requested, attached is our webpage with information detailing the fees, <br /> forms, and paperwork needed for NEW sites to obtain a boring permit. <br /> http://www.sjgov.org/ehd/docs/Requ_irements%20for%20Unit%20IV%2OWell% <br /> 20Permit.pdf <br /> as you will read, paragraph #3 details the $375 fee amount along with the Master <br /> File Record form (MFR) requirement for NEW sites to enter into our program. This <br /> is in addtion to the permit fees/form requirements detailed in paragraph #4. <br /> 1 have checked our database to verify that this site was not already listed as a <br /> current site under investigation or if a billing party has been/is established with us. <br /> Unfortunately, this site is not active on our database or accounting files. It WAS an <br /> active Department of Toxic Substances (DTSC) site previously, but it received it's <br /> NO FURTHER ACTION letter (site closure) in March 1997. Closed sites are not <br /> active sites. Any work at a closed site is considered a new site and must adhere to <br /> the information and requirements listed in paragraph #3. <br /> 1 can only surmise that Mr. Knoll misunderstood Charles or that the site was <br /> 'closed' (NFA issued) when he advised you no $375 pre-payment or MFR <br /> was needed for you to undertake your proposed subsurface investigation. Before <br /> your paperwork can be submitted to an inspector, an additional MFR form is <br /> 9/16/2011 <br />
