Proposal for Limited Site Investigation Irerracon <br /> Accurate Air Engineering, Inc. w Lodi, California <br /> August 18, 2011 g Terracon Proposal No. 60110154 <br /> Assessment Manual (CAM) 17 metals, and polynuclear aromatic hydrocarbons (PAHs) above <br /> relevant laboratory reporting limits in the on-site soils and groundwater as a result of potential <br /> releases from the RECs identified for the site. <br /> Following receipt of your executed Agreement for Services and no later than 48 hours prior to <br /> intrusive activities, Terracon will contact a utility locator (DigAlert) to arrange for underground <br /> utility locates at the above referenced site. Terracon will additionally depend on assistance <br /> from client and/or site contact for location of private underground utilities at the site. <br /> Terracon will develop a generic safety plan to be used by our personnel during field services. At <br /> this time, we anticipate that an Occupational Safety and Health Administration (OSHA) Level D <br /> Personal Protective Equipment (PPE) Level D work uniform consisting of hard hats, safety <br /> glasses, protective gloves, and steel-toed boots will be required by all personnel in the work <br /> area. It may become necessary to upgrade this level of protection, at additional cost, while <br /> sampling activities are being conducted in the event that petroleum or chemical constituents are <br /> encountered in the on-site soils, soil gas, or groundwater, which presents an increased risk for <br /> personal exposure. <br /> The LSI report will be prepared for the exclusive use and reliance of Accurate Air Engineering, <br /> Inc., and Comerica Bank. Use or reliance by any other party is prohibited without the written <br /> authorization of Accurate Air Engineering, Inc., and Terracon. Reliance on the LSI report by the <br /> client will be subject to the terms, conditions and limitations stated in this proposal, the LSI <br /> report, and our agreement. Reliance on the LSI report by Comerica Bank will be subject to the <br /> terms contained in Comerica's Environmental Consultant Agreement (Master Agreement) with <br /> Terracon (September 12, 2003). The limitation of liability defined in the Master Agreement is <br /> the aggregate limit of Terracon's liability to Comerica Bank. The limitation of liability defined in <br /> our Agreement for Services with of Accurate Air Engineering, Inc., is the aggregate limit of <br /> Terracon's liability to the client and other relying parties other than Comerica Bank. <br /> Soil Borings and Temporary Groundwater Sampling Points <br /> A total of six soil borings will be advanced on site through the warehouse floor (SV-1 and SV-2 <br /> to a proposed depth of five feet below grade surface [bgs]), near the clarifier (B-3 to a proposed <br /> depth of eight feet bgs and SV-4 to a proposed depth of five feet bgs), near the waste oil <br /> storage pad (B-5 to a proposed depth of five feet bgs); and near the former coal storage area <br /> (B-6 to a proposed depth of five feet bgs).
