S A NJ O A Q U I N Environmental Health Department <br /> --COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> POLY PROCESSING COMPANY LLC 8055 S ASH ST, FRENCH CAMP May 19, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. <br /> In the hazardous waste storage area: <br /> -One black 55 gallon steel drum containing used oil(full)had a hazardous waste label that was missing the <br /> hazardous property. <br /> -One black 55 gallon steel drum containing used oil(1/3 full)had a hazardous waste label that was missing the <br /> hazardous property and had an incorrect accumulation start date. According to Paul Thrailkill, Maintenance <br /> Manager,the correct date was 8/26/2019. <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: Paul corrected the violation by fixing the labels during the inspection. No further action is <br /> required to correct this violation. <br /> This is a repeat violation, Class II. <br /> 608 CCR 66266.130 Failed to properly manage used oil and fuel filters. <br /> OBSERVATION: Paul Thrailkill, Maintenance Manager, stated that Done Right Field Service services the forklifts <br /> on site and takes used oil filters back with them. Disposal records for the used oil filters for the last three years <br /> were not found on site. <br /> REGULATIONS: Per Title 22 California Code of Regulations(CCR)66266.130,oil filters that are not completely <br /> drained need to be managed as <br /> hazardous waste. If the oil filters are completely drained,they can be disposed of via recycling.A bill of lading needs <br /> to be left with the facility for drained oil filters. Per 22 CCR 66266.130(c)(5), bills of lading must indicate generator, <br /> transporter, and receiving company names, addresses,telephone numbers,the quantity and size of used oil filter <br /> containers transferred,and the date of transfer. <br /> CORRECTIVE ACTION: If the oil filters are being taken as recycling, immediately start to keep and store the bills of <br /> lading when the filter waste is generated. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by June 18, 2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign.
