Also, the below information is required for each nominated <br /> site; <br /> • site name and address; <br /> • name and address of all identified potentially responsible <br /> parties; <br /> • a copy of the final corrective action order for each <br /> recalcitrant site; <br /> • the name and address of the oversight agency (the agency <br /> requesting the direct site funding) , including the name and <br /> telephone number of the regulatory contact person; <br /> • a description of the unauthorized release, including the <br /> water body affected or threatened, the threat to water <br /> quality, and any threat to human health, safety and/or the <br /> environment; <br /> • a description of why the responsible party cannot or will <br /> not cleanup the contamination or abate its effects; <br /> • documentation of any disputes between the regulatory agency <br /> and the responsible party; <br /> • a description of the proposed cleanup or abatement; <br /> • the amount of annual funding requested and an estimated <br /> total required funding for each nominated site; and <br /> • a description of the results if funding should be denied. <br /> It is important to remember that funding is limited.
