Hi Renee,
It seems that this is what I need to issue this permit.
1. We need $375. You sent $500 which covers the MFR fee ($375) and the Permitting fee
($125), but there is a 3 hour prepayment for inspections ($375) that still must be received
before the moneys are correct.
2. There must be a signed letter from the property owner that says that he agrees to let you
onsite, to do drilling and that he is aware that if chemicals are found he will be placed on
the state registry and that he will ultimately be responsible for the clean up of the
contamination.
3. The grouting requirements of this county require neat cement to fill the borings.
4. The permit was not signed. It must be signed by a person, with authority to sign, by
Antea Group.
5. Was the letter sent to Nuel on March 6 2012 considered to be the work plan? A work
plan is required. It should have a history and at least 2 maps. A health and safety plan
is usually part of the work plan. The chemicals that you are looking for and the methods
used to detect these chemicals are also part of a work plan.
If you have any questions call me at (209) 468-3147, or just e-mail me.
Johnny Yoakum
Senior REHS
San Joaquin County
Environmental Health Department
From: John Yoakum [EH]
Sent: Wednesday, March 14, 2012 4:22 PM
To: 'renee.ransom@anteagroup.com'
Subject: FW: Well & Boring Permit Application - 908 E Scotts Ave, Stockton, CA
Hi Renee
We need $375, an access agreement from the property owner and final grout must be neat
cement.
Johnny
From: Lisa Medina [EH]
Sent: Wednesday, March 14, 2012 4:12 PM
To: John Yoakum [EH]
Subject: FW: Well & Boring Permit Application - 908 E Scotts Ave, Stockton, CA
FYI
3/28/2012
