PERMIT <br /> Piedmont,AL 36272 /SERVICES <br /> UNDERGROUND STORAGE TANK CLEANUP FUND(FUND),REQUEST FOR FURTHER <br /> DOCUMENTATION DURING INITIAL REVIEW: CLAIM NUMBER 017360;FOR SITE <br /> ADDRESS: 433 SCOTTS AVE W, STOCKTON <br /> After reviewing your claim application to the Cleanup Fund,we find that the following additional <br /> information is needed to determine your eligibility for placement on the Priority List: <br /> ❑ Claimant is requested to submit a copy of the UST Unauthorized Release(LEAK) Contamination <br /> Site Report(URF). Claimant's letter of July 24,2002 indicates claimant did not have an <br /> unauthorized release of petroleum(which if true would make this claim ineligible). However, San <br /> Joaquin County's letter of April 16,2002 indicates that an URF was filed. Please submit a copy of <br /> that report. <br /> ❑ Claimant has indicated in application that it is only the property owner(which would not qualify you <br /> as an eligible claimant). It appears that claimant may be a De Facto UST owner as described on Page <br /> seven(7)of the claim application instructions(copy enclosed).
