Advanced <br /> • GeoEnvironmental, Inc. r � <br /> 04 DEC <br /> 01 December 2004 <br /> AGE-NC Project No 02-0950 <br /> Ms Margaret Lagono <br /> San Joaquin County Environmental Health Department <br /> 304 East Weber Avenue, Third floor <br /> Stockton, California 95202 <br /> Subject: Sensitive Receptor Survey <br /> PREM DOR <br /> 433 West Scotts Avenue, Stockton, California <br /> Dear Ms Lagorio <br /> At the request of Dr Sonya 01brantz of Masonite International/Prem Dor, Advanced <br /> Geo Environmental, Inc (AGE)has conducted a sensitive receptor survey as part of the initial site <br /> characterization requirements under the California Code of Regulations - Title 23, Division 3, <br /> Chapter 16, Section 2654 for the site located at 433 Scotts Avenue in Stockton, California <br /> OBJECTIVE AND SCOPE OF WORK <br /> The objective of the survey was to identify and locate sensitive receptors,including wells,potentially <br /> affected by the UST release,and to identify and locate potential vertical conduits The scope of work <br /> included the following tasks <br /> 1 Review of State of California Department of Water Resources records, <br /> 2 Review of San Joaquin County Department of Health Services records, <br /> 3 Performance of a visual area reconnaissance for municipal, domestic, irrigation or ground <br /> water monitoring wells within a 2,000-foot radius of the site <br /> In addition,local utility service companies were contacted to confirm water and sewer service in the <br /> area of the site, and locations of municipal wells The location of the site, and the immediately <br /> surrounding area are illustrated on Figure 1 Site structures are depicted on Figure 2 <br /> 837 Shaw Road,Srockton,California 95215 <br /> Telephone (209) 467-1006 FAX(209)467-1118 <br />
