SIERRA NEVADA; STOCKTON <br /> r, GRouNDWATER ASSESSMENT ACTIVITIES <br /> 'L <br /> We are in receipt of your consultant's request for additional <br /> subsurface investigations for the above referenced site. To date, we <br /> have not received the completed report for the assessment activities <br /> that were performed March 8, 1988. Submit the completed report as <br /> prepared by Groundwater Technology, Inc. within the seven (7) days from <br /> the date of this letter. Please be advised that concurrent submission <br /> of this report should also be made with the Central Valley Regional <br /> Water Quality Control Board. <br /> Pursuant to the San Joaquin Local Health District fee schedule <br /> {) Resolution No. 85-58, a $35.00 consultation fee shall be submitted <br /> 1 along with the completed general Environmental Health application form <br /> for review of the above referenced environmental assessment/remediation <br /> activity. <br /> Each additional hour over the initial one hour of time spent in review <br /> activity will be billed at the rate of $35.00 per hour. The additional <br /> amount due will be billed to the applicant identified on the general <br /> Environmental Health application format the completion of requested <br /> activity, or monthly, if longer than 30 days is required for <br /> completion. <br /> In the event groundwater contamination is documented through subsequent <br /> and/or current investigations, all environmental assessment/remediation <br /> activities shall be reviewed by the Central Valley Regional Water <br /> Quality Control Board as the primary regulatory agency. The San <br /> Joaquin Local Health District will continue its involvement for the <br /> issuance of any and all local permits for subsurface environmental <br /> investigations with concurrent review with the CVRWQCB, and receipt of <br /> written comments from the CVRWQCB. <br /> Should you have questions concerning this matter, contact Diane M. <br />
