The following is an itemized list of hazardous waste violations that have not been <br /> addressed for YANG SEE YEE AUTO SERVICE as of March19, 2020. <br /> Open violations from July 19, 2019 inspection <br /> Violation #114-Consolidated manifest receipts failed to contain complete information. <br /> OBSERVATION The consolidated manifest receipt for used oil was missing the following required information <br /> correct name and EPA ID. The consolidated manifest was under Jimmy's Auto Repair with EPA number: <br /> CAL000287119 sent out 10-19-18. Current facility name is Yang Seeya Auto Repair. <br /> REGULATION: All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA) identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported- <br /> -The <br /> ransported;-The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> CORRECTIVE ACTION: Please update or obtain an ID number to manage hazardous waste and submit evidence <br /> to the EHD. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #119-Failed to report program data in the California Environmental Reporting System (CERS). <br /> OBSERVATIONS A submission to the California Electronic Reporting System (CERS) for the hazardous waste <br /> program has not been made. <br /> REGULATION: Beginning January 1, 2013, all businesses are required to submit all new(or any changes to <br /> existing) hazardous waste information online to the CERS at http://cers.calepa.ca.gov. <br /> CORRECTIVE ACTION: Immediately create a CERS account and make the required submittals. Be sure to include <br /> your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant activities <br /> and required fields. <br /> Page 2 of 5 <br />
