O D <br />DATE: 5-10-88 <br />RE: Simplot Critique with Emergency Responders and Field Investigation <br />Unauthorized Release of 4 May 88 10:05 PM <br />16777 S. Howland Rd., Lathrop, CA <br />Critique Meeting <br />Simplot experienced a 13 -minute emission of an unauthorized release <br />of sulfur trioxide (SO3) 4 May 1988 sometime around 10:00 PM. <br />Manteca P.D. patrol observed and drove through a "fog" at the <br />intersection of Louise Avenue and N. Union Street, The patrolman's <br />estimation of the "fog" cloud size was "2 mile wide x 12 miles <br />long floating above the ground like fog".1 <br />Thirty minutes of time elapsed prior to Manteca/Lathrop Rural <br />Fire Department notification and U.E.S. notification. By the <br />time, M/L Fire responded to incident scene, the vapor gas cloud <br />had traveled easterly across the city of Manteca and was encountered <br />by motorists2 on Hwy 99 South of Lathrop Rd., North of the 120 <br />Bypass. <br />Discussion in the meeting 5 May 88 centered around Simplot's lack <br />of emergency notification via 911. Simplot's emergency notifica- <br />tion process will be adjusted to provide immediate notification <br />to O.E.S. via the 911 emergency number. <br />Field Investigation <br />The Merck Index indicates the substance released is an acute <br />irritant and may cause coughing, choking and severe discomfort <br />at concentrations of 1 ppm -3 <br />1. Lucky's Market; Wayne Sanford, Store Manager stated two <br />night shift employees suffered minor eye irritation. The A/C <br />units were functioning and the vapor gas cloud passed through <br />the store. <br />2. Jack in the Box: Interviewed manager and she state three <br />employees went to Manteca Hospital for treatment. Their <br />symptoms were: difficulty in breathing and coughing. <br />Hospital recommended taking a cold shower. One of the employees <br />was was available for questions. She told me that persons <br />from the Shell Station across the street were also exposed. <br />3. Shell Station: Owner stated no employees complained. <br />Left my card for any follow-up that may occur. <br />
