The following is an itemized list of hazardous waste violations that have not been <br /> addressed for ALL STAR HEAVY HAUL & TOW as of Januaryl0, 2020. <br /> Open violations from October 15, 2019 inspection <br /> Violation#106-Failed to train employees on waste handling and emergency procedures. <br /> OBSERVATION At the time of inspection, it could not be demonstrated that employees who handle hazardous <br /> waste were property trained. <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with proper <br /> waste handling and emergency procedures, relevant to their responsibilities during normal facility operations and <br /> emergencies. <br /> CORRECTIVE ACTION: Immediately train the employees. Provide proof of training within 30 days to the San <br /> Joaquin County Environmental Health Department (EHD)for employees whose responsibilities include hazardous <br /> waste. <br /> ❑ This violation was corrected `]This violation will be corrected by (date): L pmt] <br /> ❑ Supporting documents included <br /> Describe actions taken or will betaken to correct violation: f YAA�� <br /> vlcb1ICIasS <br /> Violation #113-Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION Copies of consolidated manifest receipts for hazardous wastes including used oil, and household <br /> hazardous waste were not found on site during the inspection <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action regarding <br /> the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Immediately locate a copy of all consolidated manifest receipts from the past three years <br /> and maintain them on site.
