Extraction Well Installation <br /> Summary Report and Injection <br /> ARCADIS Well Installation Work Plan <br /> Former Occidental Chemical <br /> Corporation Facility <br /> Lathrop, California <br /> • The current property owner, Simplot,was contacted to discuss the presence of utilities in the vicinity <br /> of the proposed work. <br /> • Underground Service Alert was contacted in accordance with local notification requirements. <br /> • A private utility locator was contracted to assess the presence of potential underground utilities in <br /> the vicinity of the boring locations. <br /> • Each boring location was cleared to a depth of approximately 5 feet below ground surface(bgs) <br /> using an air knife. <br /> • The Site-Specific Health and Safety Plan, dated September 23,2011 (ARCADIS 2011), outlining <br /> the general health and safety procedures followed at the Site was reviewed by ARCADIS field staff <br /> and the subcontractor's field staff. Potential health and safety concerns specific to the proposed <br /> investigation activities are addressed in the Standard Operating Procedures provided in Appendix B <br /> of the Site-Specific Health and Safety Plan. <br /> 2.2 Field Activities <br /> The field procedures summarized in this section were implemented to complete the well installation <br /> activities in accordance with the work plan(ARCADIS 2012a)and addendum (ARCADIS 2012b).Wells <br /> were completed in accordance with Section 4.7 of the SJCEHD well standards, which is included in <br /> Appendix A. <br /> Well installation activities were conducted by Cascade Drilling, L.P. (Cascade)of Rancho Cordova, <br /> California using a mud rotary drill rig [for wells EW-12A, EW-12B, EW-12B(R), EW-13, EW-14A, EW- <br /> 14B, EW-15A, EW-15B] and a hollow-stem auger(HSA)drill rig(for wells EW-10, EW-11,and EW-17), <br /> which operated under the oversight of an ARCADIS geologist from December 19, 2012 through March <br /> 23, 2013.Well construction logs are presented in Appendix B. <br /> Extraction wells EW-12A, EW-14A, and EW-15A were constructed with screened intervals within the <br /> shallow and intermediate depth intervals. EW-13 was screened in the intermediate groundwater zone. <br /> Extraction wells EW-12B(R), EW-1413, and EW-15B were screened in the deep groundwater zone. EW- <br /> 10, EW-11, and EW-17 were screened in the water-table groundwater zone.
