13.14.4. For aboveground completions, a 2 ft X 2ft concrete surface seal shall be <br /> constructed around the top of a monitoring well casing at ground <br /> surface at least four(4) inches thick and shall slope to drain away from <br /> the well. The depth may extend three (3)feet below the ground surface <br /> and the seal shall be continuous with the annular seal and the well <br /> cover. <br /> 13.14.4.1. The surface seal should be free of cracks, voids, and <br /> other significant defects that would allow the entry of <br /> water, pollutants, and contaminants. <br /> 13.14.4.2. The joints between the surface seal, the annular seal, and <br /> the well casing shall be designed and built so that they <br /> will not fail, or cause the failure of the well casing or <br /> annular seal, and remain watertight. <br /> 13.14.5. Permanent protective barriers or the equivalent shall be installed to <br /> house the top of a monitoring well where the well casing must be <br /> terminated above ground surface. <br /> 13.14.6. A structurally sound, watertight vault, or equivalent, shall be installed to <br /> house the top of a monitoring well where the well casing must be <br /> terminated below ground surface. <br /> 13.14.6.1. The barriers shall be easily seen and shall protect the well <br /> from vehicular impact. <br /> 13.14.6.2. The annular seal shall contact the subsurface vault. <br /> 13.14.6.3. The joint between the vault and the annular seal, and the <br /> vault and the well casing, if any, shall not fail or cause the <br /> failure of the annular seal or well casing, and shall be <br /> watertight. <br /> 13.14.6.4. Appropriate sealing material shall be placed between the <br /> outer walls of the vault and the excavation into which it is <br /> placed. <br /> 13.14.6.5. The vault and its lid shall be strong enough to support <br /> vehicular traffic, where such traffic might occur. <br /> 13.14.6.6. The vault lid shall be clearly labeled. <br /> 13.14.6.7. A security device to deter unauthorized access shall be <br /> installed. <br /> 13.14.6.8. The top of the vault should be set at, or above, grade so <br /> that drainage is away from the vault. <br /> 13.14.6.9.
