The following is an itemized list of hazardous waste violations that have not been <br /> addressed for PROFESSIONAL AUTO REPAIR as of February03, 2020. <br /> Open violations from January 26, 2016 inspection <br /> Violation #101 - Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br /> website http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. <br /> Immediately reactivate your EPA ID number and submit evidence to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #117-Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on September 10, 2013 and an inspection report was issued identifying information to <br /> be submitted to bring this site into compliance. This information was required to be submitted by October 10 2013. <br /> This information has not been received resulting in a non-compliant status for this facility. An operator that receives <br /> an inspection report shall have 30 days to submit a written response that includes a statement documenting <br /> corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response <br /> documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br /> Violations that have not been corrected: <br /> - EPA ID number inactive <br /> - Failed to determine is a waste a hazardous waste for oily rags <br /> - Failed to completely label containers of hazardous waste <br /> - Failed to managed drained used oil filters by not properly labeling them <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #117-Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS) for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> Page 1 of 3 <br />
