1 . The rehabilitation of each phase shall be completed , in <br /> conformance with the rehabilitation plan , within two years <br /> of completion of the extraction portion of that phase . <br /> m. The applicant must pay a fee of $150 to the County to <br /> cover the cost of an annual inspection of the excavation <br /> to assure compliance with the provisions of the permit . <br /> 2 . Department of Public Works : <br /> a . The first 100 feet of the private road from the intersec- <br /> tion of Kasson Road shall be surfaced with a minimum two- <br /> inch asphalt concrete . <br /> b. An encroachment permit shall be required for work within <br /> the road right-of-way . <br /> 3 . Air Pollution Control District : <br /> The applicant shall apply for an authority to construct per- <br /> mit . <br /> 4 . Department of Public Health Services : <br /> a . The well must be repaired under permit and inspection by <br /> the Environmental Health Division. <br /> b . No excavation is permitted into the groundwater table <br /> without specific approval from the Regional Water Quality <br /> Control Board,' Central Valley Region . <br /> c . The materials from an excavation shall not be deposited <br /> into any water course nor in any way contribute to the <br /> pollution thereof . <br /> d. Provide an approved potable water supply for all <br /> employees . <br /> e . Approved chemical toilets shall be provided for all <br /> employees . Toilets must be maintained in a clean and <br /> sanitary manner .
