%..41 \.1010 <br /> 2) A site conceptual model describing beneficial groundwater use, the regional <br /> hydraulic setting, aquifer and wellhead vulnerability, groundwater as a drinking <br /> water supply, and alternatives drinking water sources. <br /> 3) A site plan showing surface waters, recharge areas, and water wells within 2,000 <br /> feet of the site. <br /> 4) Hydrologic cross-sections showing the lateral and vertical extent of impacted soil <br /> and groundwater. <br /> 5) Mass balance calculations. <br /> 6) Validation of contaminant fate and transport assumptions for the site. <br /> 7) Tabulated monitoring data collected from the site. <br /> 8) A comprehensive narrative of site corrective actions. <br /> A minimum of four copies of the report shall be produced for submittal to the City of <br /> Stockton, PHSIEHD, the SJVUAPCD, and the Regional Water Quality Control Board — <br /> Central Valley Region (RWQCB). <br /> Section 10.13 Monitoring Well Destruction <br /> Upon receipt of site closure, a workplan shall be prepared for well destruction activities. A <br /> figure showing existing wells and construction details is included in the Plans. All existing <br /> wells and any new wells installed at this site, a total of eighteen (18) wells, shall be <br /> destroyed. The workplan shall be submitted to PHSIEHD along with a permit application <br /> and associated fees. PHSIEHD generally requires that monitoring wells be destroyed by <br /> overdrilling the well casing and sandpack. The well boxes and locking plugs shall be <br /> removed from each wellhead and the casing and sandpack shall be overdrilled. The <br /> boreholes shall be subsequently grouted to one foot below the surface with neat cement or <br /> bentonite grout using tremie pipe. The top foot of the borehole shall be backfilled to match <br /> the surface. Subsurface piping shall be abandoned in place by backfilling with neat <br /> cement or bentonite grout. <br /> Section 10.14 Remediation System Decommissioning <br /> Upon receipt of site closure, the remediation system shall be dismantled. All equipment <br /> including enclosure fencing shall be removed form the site and properly disposed of. <br /> Any broken paving will be replaced with 6" asphalt concrete Class B, '/2" maximum <br /> aggregate with 5.8% asphalt concrete. Reusable equipment will be deemed to be the <br /> property of the contractor. <br /> SP12 <br />
