Environmental <br /> Mr.Michael J.Infurna Resources <br /> 16 December 1998 Management <br /> Page 4 <br /> ERM will direct the drilling subcontractor to drill out wells with more <br /> than 3 feet of silt or those that have experienced casing failure. Prior to <br /> drilling, a steel guide rod will be inserted in the well to ensure that the <br /> drill stem stays centered on the well. Hollow-stem augers (with a <br /> diameter equivalent or larger than those originally used to drill the well <br /> borehole) will then be used to drill out the grout seal, casing, and filter <br /> pack. The abandonment borehole will extend into native material <br /> approximately 2 feet further than the original borehole. The over drilled <br /> borehole will be backfilled with neat cement using a tremmie pipe as the <br /> augers are removed from the borehole to within 5 feet of the ground <br /> surface. At depths less than 5 feet bgs,the cement will be poured directly <br /> into the borehole. <br /> ERM will maintain drill cuttings (if any) generated during well <br /> abandonment procedures onsite, and will sample and dispose the <br /> materials in accordance with applicable state and federal laws. Steam <br /> clean rinsate and water displaced during well abandonment will also be <br /> stored onsite, and will be sampled and disposed in accordance with <br /> applicable state and federal laws. <br /> In accordance with permit requirements, ERM will call PHS-EHD 48 <br /> hours prior to abandonment activities to coordinate the required <br /> inspections. <br /> We appreciate your prompt attention to this proposed workplan and <br /> application.
