Were there any removal reports for these USTs? Provide a copy. <br /> • Verification from the local agency that an unauthorized release of petroleum from the UST was <br /> discovered on a specified date. <br /> • Submit a copy off the UST Unauthorized Release(LEAK) Contamination Site Report. <br /> • Copy of the first letter from the local regulatory agency naming you a responsible party and directing <br /> you to cleanup the contamination at the subject site. <br /> • On page two (2)you have listed four USTs that were removed in 1980. Please note that claimant will <br /> not be eligible for any costs associated with these USTs (if there is release associated with these <br /> USTs) because claimant never owned or operated them. Claimant didn't acquire site until 11/92. <br /> However, you have also indicated that four USTs were filled with sand and abandoned in place in the <br /> late 1960's. If these USTs were still in the ground when claimant acquired the site, then you may be <br /> an eligible claimant for these USTs. Provide some form of evidence as to the existence of the four <br /> USTs that were abandoned in place in the late 1960's(i.e. tank integrity tests, monitoring tests, <br /> inventory reconciliation or tank gauging, etc.). Also, claimant will be required to provide evidence <br /> that the release for the subject claim is associated with these USTs and not the ones removed in 1980. <br /> • Claimant has provided an "original signed" Verification and Signature Page (7) (see enclosed copy of <br /> page seven). What is signatory's title as related to the claimant? <br /> Priority Class "B" small business is for those businesses that are independently owned and operated, not <br /> dominant in its field of operation and employs 100 or less employees, including all affiliates, and <br /> averages annual gross receipts of$10 million dollars or less over the previous three years. If a business is <br /> a manufacturer, there is no revenue test.
