Installation of three onsite monitoring wells to shallow groundwater.
2. Installation of two direct push piezometer probes on-site to collect grab groundwater samples at depths of 60-feet
and 100-feet bsg.
3. Installation of one direct push piezometer probe off-site to collect soil samples and a grab groundwater sample,to a
depth of 50-feet bsg, located east of the site on the southeast corner of Minor and California Streets.
4. Completion of a well/sensitive receptor survey covering a 2000-foot radius from the site. As noted before, sources
other than the Department of Water Resources may have to be utilized in order to complete a thorough search.
CMQvIENTS AND CONDITIONS:
1. The three onsite monitoring wells must all be built to the same depth,to encounter first groundwater. This is
necessary to determine a true site groundwater gradient direction. The wells must be built with a maximum screen
length of 20 feet. The need for deeper, discretely screened wells will be determined following receipt of the
analytical data from the 60 and 100-foot bsg grab groundwater samples.
2. The proposed piezometer probe located on the northwest corner of Minor and California Streets (probe I) is not
required or approved at this time.
3. Analysis of all samples, soil and groundwater, must include TPHg and TPHd(EPA Method 8015m),BTEX(EPA
Method 8020) and fuel oxygenate additives, 1,2-DCA and EDB (EPA Method 8260B).
}. Submit a San Joaquin County Well Permit Application and required$89.00 permit fee for each parcel of land on
which borings will be advanced. City of Stockton encroachment permits are required for work to be done in the
city right of way,and copies must be provided to PHS/EHD before a drilling permit can be issued.
DATED BY < Q
Lori Duncan,Senior REHS (209(_) 7468-0337
CC: Marry Har-E=11,CVRWQCB
John O'Rourke,Hydro-Geo Consultants
A Division of San Joaquin County Health Care Services
