PHS/EHD has the following <br /> comments for your consideration. <br /> The addendum to the work plan proposed to install an additional monitoring well in the <br /> northeast area of the former excavation to investigate groundwater contamination which <br /> may be associated with former tank #4. <br /> As PHS/EHD indicated in correspondence dated April 4, 1994, the number of soil <br /> borings proposed will be inadequate to investigate the areas of residual contamination. <br /> Please note that care should be given to ensure that sufficient soil samples are collected <br /> and analyzed to estimate the volume of residual soil contamination. You should be <br /> aware that PHS/EHD may request additional investigation to define the extent of <br /> residual contamination. <br /> A health and safety plan must be submitted prior to initiating field activities proposed. <br /> Enclosed please find the drilling permit # 94-003509 for three soil borings, one of which <br /> will be converted into a monitoring well.
