PHS/EFID <br /> recommends that 2 samples for every 100 cubic yards be collected and composited to one sample by the <br /> laboratory for analysis. <br /> The work plan indicated that an additional 15% cost would be charged by American Environmental <br /> Network for full QA/QC documentation. Information which American Environmental Network provided <br /> PHS/EI ID, indicated that QC data was provided with each report at no additional charge. It is likely that <br /> the QC data which will be provided at no additionalcost will,be sufficient to validate the analytical results. <br /> Groun0water sampling of MW4 may be temporarily suspended pending evaluation of groundwater <br /> monitoring data from MW1, MW2, and MW3. <br /> Please submit a permit application and $89 fee along with a health and safety plan. Should.you wish to <br /> amend;the proposed work plan, please also submit a work plan addendum. Please provide PHS/EHD staff <br /> with 48 hours notice prior to initiating the work in'the plan. <br /> The resultant report shouldinclude tables of all analytical results to date.
